Risk Management and Environmental Health and Safety have recently developed a new injury reporting process which will improve accuracy and efficiency. On-the-job injuries and campus injuries that are not work related should now be reported electronically by completing the Injury or Illness Report.
The report can be found on the Risk Management website (http://riskmanagement.ua.edu/on-the-job/) and the EHS website (http://ehs.ua.edu/). The obsolete reporting form will soon be removed from the website and will no longer be accepted.
