Services: Medication Assistance Program. Eligibility Determination
for Charity Care. Assistance with Applications for Medical and Financial Benefits.
Nursing Home, Assisted Living, and Hospice Referrals. Patient Counseling. Community
Resource Referrals for Problems of Basic Living: Food, Utilities, Rent. Assistance
with Transportation to Medical Appointments. Assessment of Elderly Patients need
for services to avoid Nursing Home admission.
Hours of Operation: Monday, Tuesday, Wednesday and Friday,
8:00 AM- 5 :00 PM.
