Services: Copies of medical records may be obtained in the
Medical Records department. There will be no charge for copies of medical records
sent to other physicians’ offices. Copies of medical records for your self
are available for $1.00 dollar a page up to 25 pages, and 50 cents a page after
the first 25 pages.
Notary services for copies of medical records are available at no charge to
the patient.
