HOW TO APPLY FOR THE
RURAL HEALTH SCHOLARS PROGRAM
1. Application must be legible either typed or printed and all
sections completed.
2. Submit two letters of recommendation. One must be from
your high school counselor and the other from one of the
following persons: a math, science, English teacher, or
principal. Recommendation letters should be sent with the
application in a sealed envelope.
3. Parent’s or guardian consent is required for you to participate in this program. Please make sure that a parent or guardian
signs this form in the appropriate place.
4. Have your high school counselor or registrar send an official
transcript of your high school grades including any
standardized test scores. A copy of these test scores must
accompany your transcript. (NOTE: to be admissible to
the program students should have earned at least a
20 ACT or 950 SAT score and a 3.0 cumulative grade
point average).
5. It is your responsibility to make certain that your
recommendations and application are sent together. Make
sure that your name and the name of the person providing the
recommendation is clearly printed on the recommendations.
6. All materials (including application, test scores,
recommendations) should be in one envelope and
postmarked by March 15.
7. Mail application and recommendations to:

8. We will communicate further with you as your application is
completed and the final selections for the Rural Health Scholars
are made. You should know your status by April 30.
