Grades for the Family Medicine Clerkship are determined by:
Preceptor’s Final Evaluations 55%
Mini-board 35%
Case Presentation 10%
In addition to the Case Presentation and the Mini-Board, additional supporting documentation is necessary to receive a grade, and should be presented on the final day of the Family Medicine clerkship (i.e., the day of the mini-board) in order to receive a grade. Supporting documentation includes:
- E*value patient logs
- Learning Issues ledger
- Mid-rotation evaluations (self and preceptor)
- Final rotation evaluations (self and preceptor)
If you receive a failing grade on the exam the first time, then pass it on second, you will be awarded a final grade of no greater than a 79 numerical score.

Student’s Self-Evaluation
Mid-Clerkship Self-Evaluation
At the middle of the clerkship, you should complete the Student’s Mid-Clerkship Self-Evaluation form. You will then discuss your evaluation and your plan for the remainder of the clerkship with your preceptor. Your self-evaluation skills will be improved by comparing your assessment with that of your preceptor. These forms, with comments and the signature of your preceptor, should be given to the clerkship director by the final day of the block. The rating you record on your Mid-Clerkship Self-Evaluation will not affect your final grade. You will gain the most benefit by using the evaluations to progress in your ability to monitor your own performance and to generate discussions with your preceptor about your performance.
Final Self-Evaluation
On the Student’s Final Self-Evaluation form, you will evaluate your own skills and performance over the entire clerkship just as your preceptor has done. Your ability to identify your personal strengths and weaknesses should have developed considerably during the Family Medicine Clerkship. This component of the overall course evaluation allows you to record your current assessments of your skills and knowledge and the improvement you have made in those areas. This form should be given to the clerkship director by the final day of the block.
Wrap Up
Once grades for the clerkship have been finalized, the student will receive a letter via mail with the final grade enclosed. The clerkship director will provide a wrap-up session with the student if requested by the student or the director.
